    An RN Home Health nurse must be able to multitask and handle numerous responsibilities throughout each day. They must also be effective communicators with patients, families, and doctors and assist patients with basic needs such as bathing and dressing, as well as more specialized care, such as wound care, medication management and IV therapy.  Our HH RNs may work with one patient on a long-term, full-time basis, or they may visit multiple patients each day. 

    • Assess patients’ condition during every visit and chart your observations
    • Perform evaluation tasks, including vital signs and medication review
    • Administer medication as prescribed by the patient’s Physician
    • Dress or redress open wounds and assess progress of healing
    • Educate patients and their families on proper home health care strategies and procedures
    • Coordinate with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient’s care plan
    • Provide palliative care as needed to keep the patient comfortable
    • Make recommendations for devices or tools that might improve the patient’s quality of life
    • Listen to the patient and respond to concerns or requests



    Provide Home Health Visits, throughout Richmond and Oakland areas; Scheduling and Chart Reviews.  Will float both Richmond and Oakland areas (Alameda County)



    • A valid professional license within the state of practice, or compact state
    • BLS from the American Heart Association
    • Proof of right to work in the United States
    • 2 years of experience in a home health setting
    • OASIS
    • Starting IV’s
    • Valid driver’s license and auto insurance



    • EPIC Charting Experience
